Then first of all check the mail.box (or mail1.box, mail2.box, etc.) file on the server to see if the message is sitting there in dead or hold state.
Secondly, is there an anti-virus package installed on the Domino server? And if so, is it a Domino-aware anti-virus solution, or is it just a standard anti-virus solution.
And third (or maybe this should be first), is the user actually set up to send through the Domino server? Or is he set up to make direct SMTP connections? (Location document, mail tab, "send outgoing mail" field.)
